For Residents: Vital Statistics

Obtaining Records

All births, deaths, marriages, civil unions and domestic partnerships occurring within the township are recorded by the Registrar of Vital Statistics thus, copies of any of the aforementioned documents may be obtained through this office.

Pursuant to the Governor’s Executive Order #18, Randolph’s Registrar of Vital Statistics may only issue a certified copy of a vital record to persons who establish themselves as the subject of the vital record, the subject’s parent, legal guardian or legal representative, spouse, child, grandchild or sibling, if of legal age.

In addition to authenticating your relationship to the subject of the vital record, you must submit proof of your identity. Acceptable forms of identification are as follows:

  • a valid photo driver’s license OR
  • a valid non-photo driver’s license plus an alternate form of ID with address (see below) OR
  • a photo ID non-driver’s license OR
  • two alternate forms of ID with address (see below)

Alternate forms of identification:

  • vehicle registration
  • insurance card
  • voter registration
  • passport
  • green card
  • county ID
  • school ID
  • utility bill

The Application

Requests for a certified copy of a vital record may be made by mail or in person. For your convenience, we have made our application  available online. In order to view it, you will need the freely available Adobe Reader software installed on your computer, version 4 or higher.

This is a “fillable form” and may be filled out on your computer. Please note that a fillable form isn’t the same as electronic filing and it is not possible to electronically submit a form—you must print it out after entering the information. Complete instructions are provided on the first page of the application.

The application and proof of your identity must accompany your request whether it is made by mail or in-person.

In Person:

Bring this application, two forms of original identification from the below list, and a check or money order made payable to the Township of Randolph.

By Mail:

Mail this completed application, photocopies of two acceptable forms of identification from the below list, and a check or money order made payable to the Township of Randolph. The address is: Township of Randolph, Dept. of Health—Vital Statistics, 502 Millbrook Avenue, Randolph, NJ 07869-3799.

Fees

Birth Certificate: $10.00

Certified copies of birth certificates may be issued for people who were born in Randolph, or whose parents where residents of Randolph at the time of the birth.

Marriage/Civil Union Certificate: $10.00

Certified copies of marriage and civil union licenses may be obtained for couples whose marriage or civil union ceremony was performed in Randolph, or for couples where one of the parties resided in Randolph at the time of their marriage or civil union.

Domestic Partnership Certificate: $10.00

Certified copies of domestic partnership certificates may be obtained by partners whose partnership was registered In Randolph.

Death Certificate: $10.00

Certified copies of death certificates may be issued for people who have died in Randolph, or who where residents of Randolph at the time of death.

Births occurring over 80 years ago, marriages occurring over 50 years ago and deaths occurring over 40 years ago are considered genealogical and therefore exact information is not required. You may provide only the name of the individual recorded on the vital record, the county where the event occurred and the year the event occurred.
