Software Requirements

To view these forms, you’ll need the freely available Adobe Reader software installed on your computer, version 4 or higher. To download the software, click on the Adobe logo above. A new window will open, allowing you to select the correct version for your particular platform.

Helpful Hints

To print out a blank form:

• Find the form you need in the list.
• Click on its link to view the form.
• Once the form is displayed in your browser window, use your browser’s print function to print it.

To download a blank form to complete/print later:

• Find the form you need in the list.
• Instead of clicking on the link, right-click and select Save Target As/Save Link As from the pop-up menu which appears.
• Save the form in the directory of your choice, then use Adobe Reader to view and print at your convenience.

To complete a fillable form on-screen and print it out:

• Find the form you need in the list.
• Click on its link to view the form.
• Further instructions are provided on the first page of the form.

Reminders

All forms submitted must be clear, legible, on good quality white paper and signed (where applicable).

Regardless of whether a form is fillable or not, you always have the option of simply printing it out and filling it in by hand.

Before submitting your form, either make a photocopy or print another copy for your records.
